Feedback makes our service better. You can raise a concern without fear of being treated differently.

Our commitment

Fair, accessible and culturally safe

You can complain in person, by phone, in writing, through a support person, or anonymously. We will offer interpreter or cultural support where it helps.

If someone is in immediate danger, call 000. Child-safety concerns should be reported through the appropriate Queensland Government reporting channel. Do not use this form to report an emergency — it is not monitored around the clock.

We confirm we have received your feedback and explain what happens next and roughly how long it will take.

We look into it

We review what happened fairly, talk with the people involved, and keep you informed along the way.

We respond and learn

We tell you the outcome, what we are changing, and how you can escalate if you are not satisfied.

Anonymous feedback

You can leave your name and contact details blank. We will still review what you tell us, though we will not be able to give you a personal response.

If you are not satisfied

You can escalate your complaint to the relevant external oversight or regulatory body. We will provide the correct contact details for your situation and support you to make that contact.
